About This Event
Join health system leaders across Ontario for two days of learning, collaboration, and idea-sharing. The Summit is hosted by the 58 Ontario Health Teams and Primary Care Networks and brings together representatives from the Ministry of Health, Ontario Health, OHTs, PCNs, Patient, Family, and Caregiver Advisors, and additional partners across the continuum of care to drive real change in integrated healthcare. This year's theme, Building a System of Care, highlights that transformation is bigger than any single organization. Better health outcomes require moving beyond organizational boundaries – where health and social organizations work collectively, share responsibility for outcomes, and align their efforts to create a more connected, coordinated, and community-driven system of care. The two-day Summit is a collective space for participants to connect with one another, learn from each other, and expand your collective impact. Sponsorship opportunities are available.
